Trade Stimulator

So I've wanted a trade stimulator for a long time now and this one found me this morning.  Works perfectly.  I looked on ebay for the paper insert for the front but couldn't find it, any ideas?

Any other markings or indications of what the manufacturer or model might be?

that is it for sure, I know what I have just not sure where to find the paper insert for the front.  I was thinking of taking an online image and having my graphic design friend make one but if someone knew of a place selling already made ones I'd just buy it

A sign shop would be probably the easiest solution

Gotta be a club or someone who collects to where you can get those... Maybe a slot machine site or group somewhere would know
